Terms and Conditions for Online Scheduling
- We are currently not accepting new patients with MEDICAID. We appreciate your consideration.
- Please be respectful to other patients and the clinical staff’s time and only book appointments at a date and time that you are able to make it to your appointment.
- If you cannot come in due to an emergency, we ask you call the office 24+ hours in advance so we are able to accommodate another patient.
- If you have insurance, please call the office in advance to provide your insurance information so we can verify it before your arrival and give you the most accurate estimates for your patient portion.
- We will contact you by phone and/or e-mail to confirm your appointment, if we cannot contact you, we cannot guarantee that your appointment time will be reserved.
